Act 1 The Flooded Depths The Dweller of the Deep is imprisoned by a random tier 1 Essence Monolith. A boat can be found past it after defeating it to be taken near the far exit of The Submerged Passage. Y
Act 1 The Climb A Legion monolith spawns near the Lioneye's Standard. Always spawns as Karui vs Eternal Empire legions, same as the historical battle that Lioneye was involved.
Act 1 The Upper Prison A switch on the column in the hallway with the crucified skeleton opens up a hidden room containing a Chemist's Strongbox, which drops several flasks.
Act 1 The Ship Graveyard Area contains a Pirate's Treasure Chest, which spawns The Drunken Corsair and two random Tormented Spirits.
Act 2 The Chamber of Sins Level 2 (Act 2) Area contains a room that contains a number of random level 1 gems.
Act 2 The Broken Bridge (Act 2) In the corner of one of the ruins contains three neutral rogue exiles - Orra Greengate, Ash Lessard, and Vickas Giantbone. Opening the nearby Wastrel's Jewellery Cache makes them hostile.
Act 2 The Fellshrine Ruins (Act 2) Area contains three "Devotional Statues". After interacting with them, a version of Vorana called The Forgotten Daughter spawns near the entrance to The Crypt.
Act 2 The Vaal Ruins Area contains a Corruption Altar that functions identically to a CurrencyVaalVaal Orb.
Act 2 The Northern Forest (Act 2) Area contains a Ritual encounter with a guaranteed Omen. Repeat visits feature the Ritual enemy packs, but no Altars.
Act 2 The Dread Thicket (Act 2) Area contains a Ritual encounter with a guaranteed Omen. Repeat visits feature the Ritual enemy packs, but no Altars.
Act 2 The Caverns Area contains a hidden room containing a crafting recipe. Accessible by clicking two interactable pillars in a room roughly in the middle of the pyramid's base floor. Y
Act 3 The Sewers

Area contains a Blight encounter. Y
Act 3 The Docks The Shipyard Terror is imprisoned by a random tier 3 Essence Monolith. Y
Act 3 The Archives Area contains a hidden room containing a crafting recipe. Accessible by clicking an interactable candle on a long wall. Y
Act 4 The Dried Lake Area contains a Legion Monolith next to the Nightwane, which has increased duration and always at least contains the Eternal faction.
Act 4 The Belly of the Beast Level 1 Area contains a Cankerous Sore with a unique enemy Heart of Corruption that drops a random Vaal skill gem.
Act 5 The Reliquary (Act 5) Two random chests in the area contain Key of Strength and Key of Devotion, used to open the vault near the waypoint. The "Vault Hoard" chest inside holds some rare items and gold.
Act 6 The Tidal Island (Act 6) Several piles of treasure can be found in the water near Riptide which drops stacks of Gold.
Act 6 The Karui Fortress A small island in the centre of the area, connected to by a red totem bridge, contains a pack of Tukohama tribe monsters from the Trial of the Ancestors. Drops a random tattoo when first killed.
Act 6 The Southern Forest (Act 6) A container called "Forest's Treasure" can be found inside a ring of trees. Spawns a pack of Plummeting Ursa and a random currency item when opened.
Act 7 The Vaal City
The Temple of Decay Level 1
Area contains an Ultimatum encounter.
Act 7 The Temple of Decay Level 1 On the first floor, instead of going straight through, taking the side passage in the centre leads to a bathed in red light; contains three loot containers with each of a corrupted rare weapon, a corrupted rare armour, and a corrupted unique weapon, all with a Vaal implicit modifier.
Act 8 The Sarn Ramparts A switch about at the base of the rampart (about halfway thorugh the area) opens a hidden tunnel to a room that contains a Smuggler's Cache and a safe.

Area contains a hidden grate; clicking it allows entry into the hidden area The Hidden Underbelly, connecting the three areas. Y
Act 8 The Grand Promenade Area contains a locked room marked by a Solaris/Lunaris mural. Activating pressure plate in each of The Lunaris Temple Level 1 (Act 8) and The Solaris Temple Level 1 (Act 8) (similar to The Twilight Temple) unlocks the door, containing a room with a "Twilight's End" chest containing a random unique item.
Act 8 The Bath House A Private Chamber can be found behind a breakable wall in the northmost corner, containing a few animated statue monsters and a chest holding 2-3x CurrencyConvertToNormalOrb of Scouring.
Act 9 The Vastiri Desert A ruined platform northeast of the Descent entrance contains three clickable statues: "Viridian Offering", "Crimson Offering", and "Sapphire Offering"; they drop a rare Viridian Jewel, Crimson Jewel, or Cobalt Jewel depending on which statue is selected.
Act 9 The Tunnel Interact with a switch on the wall in a corner of the are near the end of the Labyrinth Trial to unlock a small hidden tunnel containing a random transfigured gem.
Act 9 The Refinery Area contains an underground bunker resembling a Heist area accessible by clicking an interactable grate. Contains a small room with enemies and a crafting recipe. Y
