Map fragment
Map fragments are items that can be used in a map device to open portals to various high level areas. Placing any one of the fragments along with a map into the map device will increase the item quantity of that map.
Mechanics
Map fragment sets, as well as Breachstone and Emblems are mainly used to open portals to special map area. Fragments from sets, as well as Scarabs and Lures can be used to enhance a map when the fragments are use along with a map item. Use a single vaal map fragment can open a portal to vaal side area.
Map IIQ
If the player places a fragment along with a map into the map device, the map will have an additional 5% increased quantity of items dropped. Advanced Vaal fragments grant 10% quantity. Multiple fragments can be used to stack more quantity though they need to be of different types to stack. For example, adding 2 Sacrifice at Dusk to the map device adds just 5% item quantity where adding 1 Sacrifice at Dusk and 1 Sacrifice at Dawn would add 10% item quantity to a map area. Adding one more different fragment such as a Sacrifice at Noon would increase the item quantity to 15%.
Vaal side areas
Vaal fragments can be placed in the map device on its own to instance a random vaal side area. Adding more fragments increases the item quantity of the area, the same way it works with maps. As long as they aren't placed in the correct arrangement, a full set of fragments will open a corrupted area with 15% increased item quantity.
The level of the area will depend on the fragment used:

Sacrifice Set
The Sacrifice Set, also known as Vaal fragment set, is required to gain access to The Apex of Sacrifice, where the player can encounter Queen Atziri.
Fragments of this type can be acquired as a drop from a Vaal Vessel, from bosses of corrupted maps or from Izaro's Treasure. Additionally, a set of 4 Her Mask can be exchanged for a random Sacrifice fragment.
Mortal Set
The Mortal Set is required to gain access to The Alluring Abyss, where the player can encounter a stronger version of Queen Atziri.
The most common way to acquire them is to defeat Queen Atziri. Additionally, a set of 3 Sambodhi's Vow can be exchanged for a random Mortal fragment and a set of 3 Last Hope can be exchanged for Mortal Hope.
Pale Court Set
The Pale Court Set is required to gain access to The Pale Court.
They can be obtained by completing certain Prophecies.
Key to the Crucible
The Key to the Crucible fragments set is required to gain access to The Shaper's Realm, where the player can encounter the Shaper.
They can be obtained by defeating the Guardians of the Void.
Key to Decay
The Key to Decay fragments set is required to gain access to the Absence of Value and Meaning, where the player can encounter the "red" Elder.
They can be obtained by defeating the Elder Guardians.
Maddening Object
The Maddening Object fragments set is required to gain access to The Shaper's Realm version of the Absence of Value and Meaning, where Uber Elder and Shaper can be encountered.
They can be obtained by defeating the Shaper (red in color) and the "red" Elder (grey in color).
Map enhancers
Divine Vessel
Divine Vessels can be placed with valid map to capture the map bosses soul, which can then be used to upgrade a Pantheon power.
Scarabs
Scarabs can be placed in the Map Device to add their modifier to a map. Scarabs are usually obtained from the Immortal Syndicate Intervention safehouse.
Lures
Lures can be obtained from a crafting option from a Primal Crushclaw that exchanges an item with an Aspect skill into the matching lure.
